POSITION PURPOSE:
Performs the duties of a Certified Medication Aide (C.M.A.) according to the scope of practice established by the Oregon Administrative Regulations for the Oregon State Board of Nursing (OSBN). Duties are assigned by either a Registered Nurse or a Licensed Practical Nurse and the C.M.A. is responsible to that supervisor for all care given.
SPECIFIC RESPONSIBILITIES:
Scope of Practice as recognized by OSBN includes administering:
– Oral sublingual and buccal medications, Eye medications with the exception of eye medications to new post-operative eye residents, Ear medications, Nasal medications, Rectal medications, Vaginal medications, Skin ointments, topical medications including patches and transdermal medications, Medications by gastrostomy and jejunostomy tubes, Dosage controlled aerosol/nebulizer, Administration of PRN medications.
CMAs may administer PRN medications (including controlled substances) to stable clients according to a physician’s or nurse practitioner’s orders in the following circumstances: In response to specific client requests: Client request must be reported to licensed nurse; and client response must be reported to licensed nurse. At the direction of the licensed nurse, when: A licensed nurse assesses the resident prior to administration of the PRN medications; and a licensed nurse assesses the resident following the administration of the PRN medication.
CMAs may: Administer regularly scheduled controlled substances, Jointly witness wasted controlled substances with a licensed nurse, Count controlled substances with a licensed nurse or another CMA.
The CMA may, as an unlicensed person, provide care as delegated or assigned by a licensed nurse pursuant to the terms and conditions in OAR Chapter 851 Division 47, Standards for Registered Nurse Delegation and Assignment of Nursing Care Tasks to Unlicensed Persons.
Infection control: Uses good hand-washing, aseptic techniques and standard precautions, Adheres to Special Precautions procedures PRN, Helps to keep the unit clean (N), Stores unused supplements and medications in proper storage containers and in refrigerators as appropriate, Disposes of wastes from medications appropriately, Cleans the medication cart as needed and at the end of the hours CMA is scheduled to work.
Daily documentation: Completes initialing of medications given, Completes documentation of any medications refused, Completes documentation of outcomes of PRN medications as stated by residents able to report such results, Notes general condition of residents on the twenty-four hour report sheet.
- Reports all issues outside the scope of practice to the appropriate licensed nurse supervisor
- Attends required in-service education programs, staff and department meetings
- Supports and adheres to Nursing Service and Administrative policies and procedures
- Uses the twenty-four hour nursing charting form to report to the nursing supervisor and the next shift of nursing staff
- Attends start-of-shift report and contributes by reading information to other staff as appropriate
- Counts narcotics at start and end of shift with other staff according to Nursing Service policies and procedures
- Cooperates and offers help to all personnel.
- Works with residents and families, answering questions and referring questions to the nursing supervisor as appropriate
- Answers pages promptly.
- Conforms to facility dress code.
- Remains available and accessible at all times when on duty except when actively providing care/medications to a resident
- Is aware of the Resident Bill of Rights and policies to prevent resident abuse.
- Other duties as assigned
Knowledge: Requires considerable practical experience and knowledge of nursing services in long term care (E). Must have CMA Certification from the state of Oregon before being able to practice as a CMA in the Health Center setting (E). Must be CPR certified within the first year of employment.
Independent Judgment: Requires a middle level of independent judgment in order to administer medications. Use of good judgment and common sense in providing resident care. Uses good judgment in determining when to report issues to the licensed nurse.
Relations with Others: Job role requires substantive interactions with residents, other staff, and frequently, with residents families. Requires a high level of tact and judgment to maintain positive relationships.
Working Conditions: Moderate to heavy physical conditions. On feet most of the time (E). Involves heavy lifting if transferring residents of over 50 pounds (N). Involves pushing, pulling, maneuvering medication cart across uneven surfaces (E). Involves walking, bending, stooping, twisting, reaching, carrying, pushing, pulling and handling (E). Occasional working on irregular surfaces and at varying heights (E). Exposure to uncomfortable temperatures or odors.
Impact of Actions: Actions affect the quality of care delivered to residents. CMAs provide essential medications and related care to residents. Actions involving documentation of medications given provide essential information to other disciplines providing health care to the residents. Actions influence the facility’s ability to meet state and federal guidelines for caring for residents.
Supervisory Responsibility: First level supervision. May give directions to CNAs to provide information the CMA needs to give care to residents.
