  • Students are required to submit their OSBN License Application and processing fees, and a copy of their Certificate of Completion from Caregiver Training Institute to the OSBN for board testing before getting their fingerprint-based criminal background check.
  • All Fingerprint information and codes are now found in the OSBN License Application for testing directions on page 1.
  • The link and codes contained in the OSBN License Application to the Fieldprint website are unique to your application and cannot be used by any other applicant.
  • When on the Fieldprint’s website, click “Schedule an Appointment,” and you will be prompted to create a secure user account. Enter your email address under “New Users/Sign Up” and click the “Sign Up” button. Follow the instructions for creating a Password and Security Question and then click “Sign Up and Continue.” You will be prompted to enter all demographic information required by the State of Oregon.
